Contact Druzak Medical Customer Service and receive a customized fax-order form that lists the items that you purchase. (724) 375-1770.
Select the items you would like to purchase by noting quantity and unit of measure (U/M).
Then, Fax your completed fax-order form to the Druzak Medical Customer Service Department at (724) 375-1775.
Faxing Hints & Tips
If you're not using a Druzak Medical fax-order form, please include a coversheet that includes the following:
Your Name, the name of your Organization or Facility, a Phone Number, your Account Number, and notation directing the Fax to the attention of the Druzak Customer Service Department.
Fax machines transmit in black & white at a low resolution. To ensure accuracy of your order, Write legibly, using a dark ink.
